Our Vision

Kent County Health Connect (KCHC) will contribute to a healthier Kent County by reducing the number of people who experience death and disability from disease due to:

  • Tobacco Use
  • Obesity
  • Heart Disease and Stroke


Tobacco Free Living
Create tobacco free city parks to protect children from the dangers of secondhand smoke.

Healthy Eating
Increase the availability of affordable, healthy foods in your neighborhood by offering healthy options at corner stores and YMCA Veggie Van locations.

Active Living
Increase opportunities for being active by establishing walking routes in the Hope Zone Neighborhoods.

Community/ Clinical Linkages
Increase access to chronic disease prevention programs such as the Diabetes Prevention Program and the number of health care providers who prescribe non-medical treatment, like physical activity or healthy eating, for patients with chronic disease.

The Goal

To reduce the major preventable chronic diseases such as heart disease, stroke and diabetes by increasing access to tobacco free environments, access to healthy foods, and opportunities for physical activity and chronic disease prevention, risk reduction and management especially among populations with health disparities.

Our goal will be accomplished by the collective efforts of dedicated local stakeholders who have joined to form Kent County Health Connect. The collective efforts of nonprofits, local governments, civic associations, and businesses will ensure that we identify local solutions and provide long-term sustainability to our Racial and Ethnic Approaches to Community Health- REACH efforts.
