Tobacco Free Living
Create tobacco free city parks to protect children from the dangers of secondhand smoke.
Increase the availability of affordable, healthy foods in your neighborhood by offering healthy options at corner stores and YMCA Veggie Van locations.
Increase opportunities for being active by establishing walking routes in the Hope Zone Neighborhoods and agreements with city parks to allow community members to use park space (Including indoor facilities) for physical activity programming.
Community/ Clinical Linkages
Increase access to chronic disease prevention programs such as the Diabetes Prevention Program and the number of health care providers who prescribe non-medical treatment, like physical activity or healthy eating, for patients with chronic disease.
